Digital consent forms

All three consents must be e-signed before we can proceed. Your typed name is your legal signature.

Media Release Consent

I permit Tumble International Montessori to photograph or record my child during activities, and to use those images inside private parent channels, printed portfolios and (with faces blurred where possible) marketing materials. I may withdraw this consent at any time in writing.

Field-Trip & Outing Consent

I authorise the centre to take my child on supervised local outings, nature walks, and pre-approved excursions with qualified staff, following the centre's transport and safety SOPs. I will be notified in advance of each trip and may opt out on a per-trip basis.

Medical & Emergency Care Consent

In case of illness or injury, I authorise trained staff to administer basic first aid and, if needed, to arrange emergency medical treatment or transport to a hospital while attempting to contact me. I confirm the allergy, medication and emergency-contact details I provide are accurate.
